What to know about a bad scalp hair tattoo

Scalp micropigmentation places pigment as small dots to imitate closely cut follicles or reduce the contrast of visible scalp. An unsatisfactory result may be cosmetic, but new inflammation after treatment needs separate attention.

Dots that are too large, a hairline that does not suit the face, patchy density or colour that clashes with nearby hair can make the result conspicuous. Pigment may also soften, spread or change tone as the skin heals and ages. Early darkness and mild tenderness can be part of healing, so the final appearance should not be judged while crusting or swelling is still present.

Persistent heat, increasing pain, pus, marked swelling or redness extending beyond the treated area is not simply an aesthetic problem. It may reflect infection or a reaction to pigment or aftercare products.

Photograph the scalp in even daylight, note when the treatment was performed and keep the pigment and aftercare details. Do not pick crusts, scrub the area or attempt to cover it with another tattoo while the skin is inflamed. Follow the original wound-care instructions unless a clinician advises otherwise.

Once the skin has settled, an experienced practitioner can assess whether careful colour balancing or added density might help. Removal or lightening may require a clinician familiar with tattoo pigment because pigment colour, depth and skin tone affect the response and the chance of texture or colour changes. A hair transplant addresses hair distribution rather than tattoo pigment, so it solves a different problem.

Arrange prompt medical assessment for worsening pain, spreading redness, warmth, pus, fever, tender swelling or skin breakdown. Seek urgent help for trouble breathing, faintness, or rapidly developing swelling of the face, lips or tongue.

A dermatologist can distinguish infection, irritant dermatitis, allergic inflammation and scarring from ordinary healing. Cosmetic dissatisfaction without these warning signs can be reviewed after the scalp has recovered.

Ask whether the skin is fully healed, whether inflammation or scar tissue is present, and whether correction, lightening or camouflage is realistic. Also ask how your pigment colour and skin tone affect the options, what unwanted colour changes could occur, and who will manage a reaction if one develops.
