The short answer

Pain after sprinting or changing direction may involve an adductor muscle, while deep pain with hip bending, twisting, clicking, or stiffness can arise within the hip. Sudden unexplained pain or symptoms involving the abdomen, genitals, or urinary system need medical assessment.

Where groin pain may originate

Hip impingement and a hip labral tear can present with groin pain during deep flexion or rotation; catching or clicking may accompany a labral problem. Hip osteoarthritis can present with groin pain and stiffness, often with reduced hip movement. These patterns overlap, so symptoms cannot establish the diagnosis alone.

Other possibilities include muscle or tendon injury, a hernia, urinary or kidney problems, and gynaecological or testicular conditions. Note any lump, swelling, fever, nausea, urinary change, vaginal bleeding, testicular symptoms, abdominal pain, or inability to bear weight.

Reducing strain while you arrange care

Stop sport or movements that cause sharp pain and avoid repeatedly testing the painful range. Comfortable walking and gentle hip movement may be reasonable if you are steady and symptoms do not increase. A wrapped cold pack can soothe a recent soft-tissue injury, but do not press directly over a tender lump or swollen genital area.

Schedule an assessment if pain affects walking, persists, repeatedly returns, or causes catching and restricted hip motion. Sports physiotherapy can guide rehabilitation after urgent abdominal, pelvic, and joint causes have been considered.

Symptoms requiring urgent evaluation

Seek emergency care for sudden severe testicular pain or swelling, a painful groin lump that cannot be gently reduced, severe abdominal or pelvic pain, fainting, heavy vaginal bleeding, or groin pain when pregnancy is possible. Fever, repeated vomiting, or rapidly worsening swelling also calls for urgent assessment.

After injury, prompt evaluation is needed if you cannot bear weight, the hip looks deformed, the leg becomes numb or cold, or pain follows a substantial fall or collision. Do not continue sport through these signs.

Preparing for a focused evaluation

Ask whether the source appears to be the hip joint, adductor muscles, abdomen, pelvis, or urinary system; which examination or imaging may distinguish them; and what activity is safe meanwhile. Explain the trigger, exact location, mechanical symptoms, and all genital, menstrual, bowel, or urinary changes.
