Bacterial cheilitis: signs, care and when to get help

Possible features include painful redness, swelling, crusting, splits or oozing. These findings can overlap with irritant, allergic, fungal and inflammation-related lip conditions, so appearance alone may not identify the cause.

The lip surface normally limits entry by germs. Dryness, lip licking, biting, saliva at the mouth corners, a wound or an existing rash can disrupt that barrier and allow bacteria to multiply.

A clinician may examine the lips, mouth corners and nearby skin, and ask about new cosmetics, dental products, medicines and habits. A swab may be considered when there is discharge, repeated infection or uncertainty about the organism. Treatment depends on whether infection is present and whether another form of cheilitis is contributing.

Keep the area clean with lukewarm water, pat it dry and avoid picking crusts. Pause fragranced lip products and do not share lip balm, towels or razors. A plain barrier product may reduce further cracking, but it does not replace prescribed treatment for infection.

Do not apply leftover antibiotics or steroid creams. The wrong product can irritate the lips, mask another diagnosis or fail to treat the cause. If a clinician prescribes medicine, use it exactly as directed and return if the area continues to worsen.

Arrange a medical assessment for increasing pain, pus, spreading redness, repeated episodes, sores inside the mouth or symptoms that do not settle with gentle care. People with reduced immunity or poorly controlled diabetes should seek advice early because skin infections may need closer management.

Get urgent help for rapidly increasing swelling, fever with spreading facial redness, difficulty swallowing, breathing trouble, tongue swelling or swelling around the eyes. These features can signal a severe infection or an allergic reaction and should not be managed only at home.

Ask what points toward bacterial infection, whether a swab is useful, which products to stop, how to protect the lip barrier and what change should prompt reassessment. Mention recent dental work, new toothpaste or cosmetics, lip licking and previous treatments.
