Arrange care when acne is deep, painful, draining, rapidly worsening, or leaving new raised or indented areas. Earlier control can reduce continuing tissue injury. A scar that grows beyond the original spot, itches persistently, or becomes painful may be a thickened scar that needs a tailored plan.

Prompt assessment is appropriate for fever, spreading redness, warmth, or severe tenderness around a lesion. These features can indicate infection rather than routine scar formation.
