Call emergency services for shoulder-blade pain with chest pressure, severe shortness of breath, fainting, sudden sweating, or pain spreading to the jaw or arm. Urgent assessment is also appropriate after major trauma or when new arm weakness is substantial or worsening.

Arrange prompt review for fever, a painful blistering rash, breathing-related pain, persistent night pain, progressive numbness, or symptoms that do not improve with activity adjustment. These features require assessment beyond a presumed muscle knot.
