What the pain may mean

A dull ache that begins after unfamiliar lifting, rowing, throwing, or pressing may come from fatigue or a mild strain in the muscles behind the shoulder blade. Pain during arm elevation, reaching behind the body, or lying on that side can also occur when rotator-cuff tendons or nearby tissues are irritated.

Neck pain, tingling, numbness, or discomfort travelling down the arm points to possible nerve involvement rather than an isolated shoulder problem. A sudden injury with a pop, marked weakness, deformity, or inability to raise the arm deserves prompt assessment.

What to do now

Pause the movement that provoked the pain and keep the shoulder gently moving within a comfortable range. For the next few days, reduce load, repetitions, and overhead work rather than repeatedly testing the painful motion. A wrapped cold pack can help after a fresh flare; gentle warmth may feel better when stiffness is the main problem.

When daily movements are comfortable, rebuild gradually with controlled technique and a lighter resistance. A physiotherapist can compare shoulder and neck movement, test strength, and adjust training if pain persists, recurs, or limits sleep and ordinary tasks.

When to seek care

Seek urgent care after a major fall or collision, or if the shoulder looks deformed, rapidly swells, becomes cold or pale, or cannot move. Get prompt medical help for new arm weakness, spreading numbness, fever with a hot red joint, chest pressure, breathlessness, sweating, or pain extending into the chest or jaw.

Arrange a routine assessment when symptoms are worsening, repeatedly return with exercise, disturb sleep, or fail to improve after a short period of modified activity.

Should I stretch the painful shoulder?

Does soreness the next day mean something is torn?
