Short answer

Spondylitis back pain treatment is individualized and commonly combines movement, physiotherapy, medicines and review of inflammatory activity.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Core approach
Treatment can combine exercise, physiotherapy, medication and ongoing clinical review.
Avoid
Extended bed rest can increase stiffness and reduce conditioning.
Track
Morning stiffness, night waking, mobility, function and symptoms outside the spine.

The treatment overview

The word spondylitis refers to inflammation involving the spine, but people sometimes use it loosely for other spinal problems. Confirming the diagnosis matters because inflammatory pain and age-related wear are managed differently. A clinician may coordinate care with physiotherapy and, when appropriate, rheumatology.

Why the pain behaves differently

Inflammatory spinal pain may be accompanied by morning stiffness, discomfort after rest and improvement with movement. Symptoms can also occur at the hips, heels or other joints, and some forms are associated with eye, skin or bowel inflammation. Those features help shape assessment; they do not establish the diagnosis by themselves.

Mechanical strain may coexist with an inflammatory condition. Your care plan therefore needs to address current inflammation, mobility, strength, work demands and any movement that repeatedly aggravates symptoms.

Components of a practical plan

Maintain regular, tolerable movement rather than prolonged bed rest. A physiotherapist can tailor spinal mobility, posture, breathing and strengthening work to your function and flare pattern. Pacing means adjusting the load while continuing safe activity, then rebuilding it gradually as symptoms allow.

Medicines may be used to reduce pain or control inflammation, but selection depends on the diagnosis, other health conditions and current medicines. Take prescribed treatment as directed and discuss poor control or side effects rather than changing the regimen independently. Sleep, smoking status and ergonomic demands are also reasonable parts of a review.

When symptoms need faster review

Contact your treating clinician when stiffness or pain is escalating, night symptoms repeatedly disturb sleep, function declines, or a medicine causes a suspected adverse effect. A painful red eye with light sensitivity, persistent bowel symptoms or a newly swollen joint should also be reported promptly because these features can alter care.

Seek emergency assessment for new leg weakness, numbness around the groin, loss of bladder or bowel control, major trauma, or severe back pain with fever and marked illness.

Questions that sharpen the plan

Ask what type of spondylitis is being treated, how disease activity will be monitored and what the goals of each medicine or exercise are. Clarify which activities remain safe during a flare, how side effects should be reported, and when referral to a rheumatologist or physiotherapist would add value.

Questions people ask

Is exercise safe when spondylitis causes back pain?

Appropriately selected movement is commonly part of care. A physiotherapist can modify intensity and technique around pain, stiffness, joint involvement and current function.

Why might I need rheumatology review?

A rheumatologist can confirm an inflammatory diagnosis, assess disease activity, check symptoms beyond the spine and guide medicines that target inflammation.
