Arrange a clinical review if back pain persists, limits walking or sleep, travels down a leg, or is accompanied by a painful abdominal bulge that does not settle. A tender bulge with vomiting or inability to pass stool or gas needs urgent assessment for a possible trapped hernia.
Seek emergency care for new leg weakness, loss of bladder or bowel control, numbness around the groin, major trauma, fever with severe back pain, or rapidly worsening pain. These are not features to manage with abdominal exercises alone.
