Back pain and ovarian cancer

Ovarian cancer can present with abdominal or pelvic pressure, bloating, feeling full sooner than expected, urinary changes, or pain that may be felt in the back. Back pain alone does not identify its cause.

Changes in the pelvis or abdomen can irritate nearby tissues or create pressure that is experienced as a dull ache in the lower back. Clinicians interpret the pain in context: where it is, whether it changes with movement, whether it is new or worsening, and which abdominal, bowel, urinary, or menstrual changes occur with it.

Muscle strain, joint problems, kidney or urinary conditions, and other non-cancer causes can also produce back pain. A clinical assessment is needed to distinguish among them.

Arrange an appointment if the pain persists, returns repeatedly, worsens, or accompanies ongoing bloating, pelvic pressure, appetite change, urinary urgency, constipation, or unexplained weight change. Record when symptoms began, their location, triggers, menstrual status, medicines taken, and any personal or family history of ovarian or breast cancer.

An examination may lead to blood tests or pelvic imaging, depending on the overall picture. Imaging findings and symptoms are considered together; no single symptom confirms ovarian cancer.

Seek urgent medical care for sudden severe abdominal or back pain, fainting, marked weakness, heavy vaginal bleeding, persistent vomiting, a swollen painful abdomen, or inability to pass urine or stool. New leg weakness, numbness around the groin, or loss of bladder or bowel control also requires emergency assessment.

Ask which features suggest a muscular, pelvic, urinary, or digestive cause; whether a pelvic examination or ultrasound is appropriate; what changes should trigger earlier follow-up; and whether your family history makes genetic counselling relevant.
