Back pain with fever: when to get help

A viral illness may cause fever and general muscle aches, but pain focused in the back can also occur with a kidney infection or, less commonly, infection involving the spine. The combination should not be assumed to be a routine strain.

Pain at the side of the back beneath the ribs, especially with burning urination, frequent urination, nausea, or chills, can fit a kidney or urinary infection. Severe local spinal pain with fever raises concern for infection around spinal structures, particularly after a recent infection or procedure, or in someone whose immune system is weakened.

Cough, breathing pain, abdominal symptoms, or a skin rash can point toward other sources. Degenerative conditions such as lumbar spondylosis can cause back pain, but fever is not an expected presentation and needs a separate explanation.

Contact a medical service promptly for advice and assessment. Tell them your measured temperature if available, exactly where the pain is, when both symptoms began, and whether you have urinary, breathing, abdominal, neurological, or skin symptoms. Mention recent surgery, injections, wounds, infections, travel, and medicines that affect immunity.

Drink fluids if you can keep them down, avoid strenuous activity, and do not use leftover antibiotics. A clinician may need an examination and targeted urine, blood, or imaging tests based on the pattern.

Go to emergency care now for confusion, fainting, severe weakness, breathing difficulty, rapidly worsening pain, repeated vomiting, inability to drink, very little urine, or a blotchy rash that does not fade when pressed. Do the same for new leg weakness, difficulty walking, numbness around the groin, or loss of bladder or bowel control.

Urgent assessment is also warranted when fever and back pain occur during pregnancy, after a recent spinal procedure, or alongside a weakened immune system. Do not delay because fever temporarily falls after medicine.
