Back pain and black stool: when to get medical help

Stool can also darken after iron tablets, bismuth-containing medicine, or certain foods, but colour alone cannot safely distinguish these effects from bleeding. Sticky, tar-like stool, a strong unusual smell, or symptoms of blood loss increase concern.

Bleeding higher in the digestive tract can turn stool black as blood is digested. Ulcers or irritation of the stomach lining may bleed, and pain from abdominal organs can sometimes be felt in the back. The two symptoms may also have separate causes, such as a musculoskeletal back problem alongside medicine-related stool darkening.

Anti-inflammatory pain medicines used for back pain can irritate the stomach and contribute to bleeding. Blood thinners can make bleeding more consequential. Lumbar spondylosis may explain recurring mechanical back pain, but it does not account for black stool and should not delay investigation of the stool change.

Seek urgent medical advice today for a new black, tar-like bowel movement, even if the back pain is manageable. Be ready to describe whether the stool was sticky or unusually smelly, when it began, and whether there was abdominal pain, vomiting, weakness, dizziness, breathlessness, or visible blood.

Bring or photograph your medicine list, including non-prescription pain relievers, iron, bismuth products, blood thinners, and supplements. Do not take additional anti-inflammatory pain medicine until a clinician or pharmacist has checked it. Do not stop a prescribed blood thinner without urgent instructions from the prescriber or emergency team.

Go to an emergency department or call local emergency services for black stool with fainting, collapse, confusion, marked weakness, severe dizziness, breathlessness, a racing heartbeat, vomiting blood or material resembling coffee grounds, or severe abdominal or back pain. These features can accompany significant bleeding or another acute abdominal problem.

Do not drive yourself if you feel faint or unstable. If black stool occurs with new loss of bladder or bowel control, numbness around the genitals or anus, or major leg weakness, emergency assessment is also required because those neurological signs can reflect spinal nerve compression.

Ask whether the stool appearance suggests digested blood, whether your medicines could be contributing, and whether blood tests or examination of the digestive tract are needed. Clarify which pain relief is suitable while bleeding is being investigated and what change should trigger immediate return.
