Back of Shin Pain: Causes, Self-Care and Warning Signs

Short answer
Pain at the back of the shin may come from an irritated calf muscle or tendon, but focal bone pain or marked swelling needs medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Location clue
- Pinpoint bone tenderness differs from a broad muscular ache and should be assessed if persistent.
- Activity clue
- Recent changes in running, jumping, hills or footwear can overload the lower leg.
- Urgent clue
- Sudden swelling, warmth or redness in one lower leg needs urgent medical advice.
What the pain may mean
The area behind the shinbone contains calf muscles, tendons, nerves and blood vessels. An ache that starts after running, jumping, hill walking or a sudden increase in training can reflect an overloaded calf or a tendon near the inner edge of the shin. Pain linked to muscle overload may feel tight or sore and may worsen when you push through the foot or stretch the calf.
A stress fracture can present with increasingly focal shin pain that is tender over a small area and worsens with impact. Pain at rest, pain at night or discomfort that persists despite reducing activity deserves assessment. Pain spreading from the back or accompanied by tingling, numbness or weakness may instead involve a nerve.
Steps you can take now
Pause running, jumping and any movement that clearly increases the pain. Choose comfortable footwear and use a wrapped cold pack for short periods if the area feels newly sore or swollen. Gentle ankle movement is reasonable when it does not sharpen the pain, but avoid forceful stretching or deep massage after a sudden injury or when the cause is unclear.
When walking is comfortable and symptoms are settling, return to activity gradually rather than testing the leg with a full workout. A clinician or physiotherapist can examine the exact tender area, compare strength and movement between legs, and decide whether imaging is appropriate.
When to seek medical care
Seek urgent medical care for sudden one-sided calf or lower-leg swelling, warmth, redness or severe unexplained pain. Call emergency services if leg symptoms occur with chest pain, breathlessness, faintness or coughing blood. These features can signal a blood clot or another urgent problem.
Arrange a prompt assessment after a direct blow, if you cannot bear weight, if the leg looks deformed, or if the foot becomes cold, pale, numb or weak. Book a routine appointment when pain keeps returning, becomes pinpoint over the bone, disrupts sleep or does not improve after reducing the provoking activity.
Questions for your clinician
Ask which structure appears to be painful, whether your examination suggests a muscle, tendon, bone, nerve or circulation problem, and whether imaging is needed. It is also useful to ask which activities are safe, what signs should stop your rehabilitation, and how to build back to work, walking or sport.

Questions people ask
Can back of shin pain come from the calf?
Yes. Calf muscle overload or strain can cause pain close to the back of the shin, especially when pushing off, climbing or stretching the calf. An examination can distinguish muscle pain from tendon, bone, nerve and circulation problems.
Should I keep exercising with pain behind the shin?
Avoid activities that reproduce or increase the pain. Low-impact movement may be acceptable if walking is comfortable and symptoms do not worsen during or afterward, but focal bone pain, limping or swelling should be assessed before returning to impact exercise.
When could back of shin pain be urgent?
Urgent assessment is appropriate for sudden one-sided swelling, warmth, redness, severe unexplained pain, inability to bear weight, or a cold, pale or numb foot. Chest pain or breathlessness alongside leg symptoms requires emergency help.
