The short answer

The muscle may feel hard or knotted during the episode and tender afterward. Exercise, prolonged positioning, dehydration, pregnancy and some medicines can contribute, although a clear trigger is not always found.

What a cramp at the back of the leg may mean

A brief tightening after activity or while resting is often a muscle cramp. Pain that repeatedly begins with walking and settles with rest needs assessment because reduced blood flow can produce a different pattern. Burning pain, tingling, numbness or weakness may point toward nerve irritation rather than an isolated muscle problem.

Lumbar spondylosis can present with leg pain, tingling, numbness or weakness when spinal nerves are affected, but cramping by itself does not establish that diagnosis. The location, triggers and accompanying symptoms help distinguish muscle, nerve and circulation causes.

What to do during and after a cramp

Stop the activity and gently lengthen the tight muscle. For a calf cramp, keep the knee straight and draw the toes toward the shin; support yourself if standing feels unsteady. Gentle massage and warmth may help the muscle relax. Avoid forceful stretching or continuing strenuous exercise through sharp pain.

After the cramp settles, walk carefully, drink fluids if dehydration is possible and note what was happening before it began. Regular calf and hamstring mobility work may help when episodes follow exercise or long periods in one position. Do not stop a prescribed medicine on your own if cramps began after a medication change.

When to seek medical care

Arrange a clinical review if cramps keep returning, disrupt sleep, persist despite stretching and hydration, or occur with numbness, weakness or walking-related pain. A clinician can review medicines, circulation, nerve function and the muscles and joints of the leg and lower back.

Seek urgent care for sudden one-sided leg swelling, redness or warmth; a cold, pale or discoloured foot; new inability to move the leg; or severe pain after an injury. Call emergency services for leg symptoms accompanied by chest pain, shortness of breath, fainting, or loss of bladder or bowel control.
