The short answer

Diffuse aching and stiffness after a new exercise are different from a sharp focal pain that starts during one movement. The second pattern deserves more caution, especially if it changes walking or spreads into a limb.

Soreness or possible injury

Post-exercise soreness often involves the muscles trained and is tender when they contract or stretch. It can follow extra volume, a new exercise or a return after time away. A strain is more likely to cause a distinct painful area, loss of comfortable movement or pain linked to a specific repetition.

Back pain has several possible sources, so location alone does not give a diagnosis. Lumbar spondylosis may present with back pain and stiffness, but clinicians use the overall history and examination rather than assuming it from workout soreness.

Support recovery

Reduce the exercise, range or resistance that provokes sharp pain. Keep up comfortable walking and routine movement instead of staying in bed. Heat can relax tight muscles, while a wrapped cold pack may soothe a recently irritated spot; never place an extreme temperature directly on skin.

Restart training with movements you can control and leave demanding back exercises until everyday bending and turning are comfortable. Increase one element at a time, such as load or volume, so you can identify what your back tolerates. Review breathing, bracing and fatigue if discomfort recurs.

When the pain needs assessment

Get urgent care if back pain occurs with loss of bladder or bowel control, numbness around the groin, new substantial leg weakness, fainting, breathing difficulty, or serious trauma. Fever with a rapidly worsening general condition also needs prompt attention.

Arrange a non-emergency review when pain fails to trend better, regularly returns despite reduced training, wakes you persistently, or is accompanied by tingling, numbness or pain travelling down a leg.
