What pain in the back flank may mean
The flank is the area between the lower ribs and the top of the pelvis, toward the side of the back. A strained muscle there may feel sore, tight or sharp, especially when you twist, bend, reach, cough or press the tender spot. It can follow lifting, exercise, prolonged posture or a sudden awkward movement.
Pain in this area does not always arise from muscle. Kidney or urinary problems can cause deeper flank discomfort and may occur with fever, nausea, painful urination or blood in the urine. Irritated spinal joints or nerves can also refer pain toward the side; lumbar spondylosis is one possible spinal condition, but the symptom pattern and examination determine whether it is relevant.

For mild pain after a clear strain, reduce heavy lifting and forceful twisting while continuing comfortable daily movement. Change position regularly rather than staying in bed. A wrapped cold or warm pack may ease symptoms; choose whichever feels better and protect the skin.

Prompt assessment is also needed after a significant fall or collision, or when pain accompanies new leg weakness, numbness around the groin, or loss of bladder or bowel control.
