Azithromycin for a skin rash: what patients should know

Many rashes arise from irritation, allergy, fungi, parasites, heat or inflammatory skin disease, and an antibiotic will not address those causes. Azithromycin can also cause a drug rash, so a new eruption during treatment needs clinical assessment.

Timing, appearance and other symptoms help separate the possibilities. A rash that begins after starting azithromycin may be a medication reaction, although another illness or exposure may be responsible. Itchy patches after contact with a product fit a different pattern from a ring-shaped fungal rash or an intensely itchy eruption affecting close contacts.

A clinician may ask when each dose was taken, where the rash began, how it spread, and whether there is fever, facial swelling, skin pain or involvement of the mouth or eyes. Photographs taken as the rash changes can help document its course.

Do not start leftover azithromycin or share someone else's prescription. If it was prescribed, take it exactly as directed unless a clinician advises a change. For a new rash during treatment, contact the prescriber promptly and ask whether to take the next dose; do not try to confirm a drug allergy by taking another dose yourself.

Keep the skin cool, avoid newly introduced cosmetics or fragranced products, and resist scratching. Before using an antihistamine or topical medicine, check that it suits your symptoms, other medicines, pregnancy status and health conditions.

Call emergency services for trouble breathing, throat tightness, faintness, or swelling of the lips, tongue or face. Seek urgent in-person assessment for blistering or peeling skin, a painful widespread rash, purple spots, high fever, or sores involving the mouth, eyes or genitals.

Arrange a prompt review if the eruption is spreading, persists after the medicine is finished, becomes warm or produces pus, or is accompanied by feeling unwell. Bring the medicine packet and a list of everything recently taken or applied.

Ask what diagnosis the antibiotic is intended to treat, whether the rash could represent a drug eruption, and what you should do before the next scheduled dose. Also ask which skin-care products are suitable and whether the reaction should be recorded in your medication history.
