Short answer

Severe heartburn is common during pregnancy, but persistent or unusual pain deserves medical assessment.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Typical feeling
Burning behind the breastbone, sometimes with a sour taste
Common timing
After eating, bending, or lying down
Medicine check
Confirm suitability with a pharmacist or maternity clinician

Answer at a glance

Pregnancy hormones can relax the valve between the stomach and oesophagus, while the growing uterus can add pressure later in pregnancy. Smaller meals, staying upright after eating, and avoiding personal trigger foods may help. Check any antacid or reflux medicine with a doctor, pharmacist, or maternity clinician because some products are unsuitable during pregnancy or can interfere with other medicines.

Why heartburn can become intense

Heartburn is a burning feeling behind the breastbone caused when stomach contents move upward into the oesophagus. It may be worse after meals, when bending, or when lying down. A sour taste, burping, or fluid reaching the throat can occur alongside the burning. Symptoms alone cannot show whether another digestive problem is present, so a clinician may assess severe, changing, or persistent discomfort.

Steps that may bring relief

Eat smaller portions and leave time between dinner and lying down. Notice whether spicy, fatty, acidic, or caffeinated foods trigger your symptoms rather than excluding many foods automatically. Sit upright after meals, avoid tight clothing around the abdomen, and raise your upper body for sleep using a stable wedge rather than a pile of loose pillows.

If these changes are not enough, ask a pharmacist or maternity clinician to select a pregnancy-compatible option and explain when to take it. Mention prenatal vitamins, iron, prescribed medicines, kidney problems, and high blood pressure so interactions and ingredients can be checked.

When to seek medical care

Get urgent help for chest pressure or tightness, trouble breathing, faintness, sweating, pain spreading to the arm, jaw, back, or shoulder, vomiting blood, or black stools. During pregnancy, severe upper abdominal pain with a strong headache, vision changes, sudden swelling, or feeling very unwell also needs prompt maternity assessment.

Arrange a routine review if heartburn repeatedly disrupts sleep, swallowing is painful or difficult, food sticks, vomiting continues, you are losing weight without trying, or self-care is not controlling symptoms.

Questions for your doctor or midwife

Ask which treatment fits your stage of pregnancy, how it should be separated from iron or other medicines, and what symptom change should trigger reassessment. You can also ask whether your pattern suggests acid reflux alone or whether an examination is needed.

Questions people ask

Can pregnancy heartburn harm the baby?

Heartburn itself reflects irritation in your oesophagus rather than direct harm to the baby. Severe pain or additional pregnancy warning signs still require assessment.

Should I take an antacid I already have at home?

Check the exact product first. Ingredients vary, and a clinician or pharmacist should consider pregnancy, other medicines, and medical conditions before recommending it.

Why is heartburn worse at night?

Lying flat makes upward movement of stomach contents easier, particularly after an evening meal. Remaining upright after eating and using a stable upper-body wedge may reduce symptoms.
