The short answer

What has happened at the elbow
An avulsion fracture occurs when a tendon or ligament pulls away a small piece of bone at its attachment. Around the elbow, the exact location matters because different attachments help control bending, straightening and joint stability.
A cast limits movement so the injured tissues can settle and the bone can heal in an acceptable position. It does not show by itself whether an injury is minor; examination and follow-up imaging guide the plan.
Protecting the arm while immobilised
Keep the cast clean and dry, and do not insert objects or powders underneath it. Elevate the arm as instructed during early swelling, move uncovered fingers regularly, and follow restrictions on lifting, sport and driving. Do not trim, reshape or remove the cast yourself.
Attend the scheduled review even if pain has eased. The clinician may check alignment, skin, circulation and nerve function, then decide when elbow movement can begin. After immobilisation, stiffness and weakness are common, so motion and loading are usually restored progressively rather than forced.
Warning signs with a cast
Seek urgent assessment if pain becomes severe or keeps increasing, fingers turn pale or blue, the hand feels cold, numbness develops, finger movement becomes difficult, or swelling does not improve with elevation. These signs can indicate pressure on circulation, nerves or swollen tissues.
Contact the treating service promptly if the cast cracks, becomes loose, gets soaked, develops a bad smell, causes a burning spot or rubs the skin. Fever, drainage or worsening redness near an exposed edge also needs review.

Ask where the fragment is located, whether the elbow is stable, what movements are restricted and when repeat imaging is planned. Clarify how to manage pain, when work or driving may resume, and whether guided rehabilitation will be needed once protection is reduced.
