Fibroadenoma size: what matters beyond the measurement

Many are small enough to be found on examination or imaging without changing the breast shape, while others become much larger. Size alone cannot confirm that a lump is a fibroadenoma or determine whether it needs treatment.

Ultrasound usually records the lump in more than one dimension and assesses its shape, margins, orientation, and internal appearance. Measurements can differ slightly between scans because of probe position and the boundary selected, so the radiologist reviews the images rather than comparing one number in isolation.

A fibroadenoma may remain stable, enlarge, or become smaller. Hormonal changes can influence its behaviour. Growth deserves reassessment, but it does not by itself establish a cancer diagnosis; the direction of care comes from the complete clinical and imaging picture.

If imaging has a typical benign appearance, the clinician may recommend observation or planned imaging follow-up. Biopsy can be used when the diagnosis is uncertain, the appearance is not typical, or tissue confirmation would change management. Removal may be considered for discomfort, distortion of the breast, substantial growth, uncertainty after assessment, or personal preference after discussing trade-offs.

Bring earlier scans to appointments whenever possible. Direct image comparison helps distinguish genuine growth from small measurement differences and shows whether the lump’s shape or internal features have changed.

Book a breast assessment for any new lump or a known lump that is clearly enlarging, becoming fixed, or causing persistent pain. Seek prompt evaluation for skin dimpling, redness that does not settle, new nipple inversion, spontaneous bloody discharge, or a new underarm lump. These signs call for assessment regardless of a previous fibroadenoma diagnosis.

Ask whether the imaging appearance is typical of a fibroadenoma, how its dimensions compare with the earlier scan, and whether any feature has changed besides size. Also clarify the reason for follow-up or biopsy, what symptoms would bring the appointment forward, and what removal would involve if the lump becomes troublesome.
