What to know about an ATS tetanus skin test

A trained clinician places a small, appropriately diluted amount in the skin and observes the site according to the product protocol. This is not a test for tetanus infection, immunity or the need for vaccination. It addresses possible allergy to a serum product made using horse-derived proteins.

A local wheal or flare is compared with the protocol's control and interpretation criteria. A reactive result alerts the clinical team to a greater concern for immediate hypersensitivity and may change the product or administration plan. A non-reactive result cannot rule out every serious reaction, so observation and emergency treatment capability remain necessary whenever equine serum is administered.

Testing practices differ because anti-tetanus products are not all the same. Human tetanus immune globulin and tetanus vaccines serve different purposes and are not assessed with this equine-serum skin test. The exact product name matters when interpreting why testing has been proposed.

Tell the clinician about previous antiserum, horse-protein exposure, asthma, severe allergies, past anaphylaxis and all medicines you take. Some allergy medicines can alter skin responses, but do not stop any medicine unless the treating team instructs you. Stay for the full observation period requested by the clinic.

Do not scratch or cover the test site unless directed. Report itching away from the site, throat tightness, dizziness, cough or breathing changes immediately. After leaving, follow the wound-care and vaccination plan because the skin test itself provides no protection against tetanus.

Call for emergency help if hives spread, the face or tongue swells, breathing becomes difficult, wheezing starts, or collapse or faintness occurs during or after testing or serum administration. Later fever, rash, joint pain or swelling after an animal-derived antiserum also warrants prompt medical advice, even when the initial skin test was non-reactive.

Ask for the exact name and source of the planned product, why passive antibodies are recommended, how the skin test will be interpreted and what alternatives are available. Also confirm whether a tetanus-containing vaccine is due and where to seek help if a delayed reaction appears.
