Surgery for atrophic scars

The word surgery covers different techniques. Subcision releases fibrous bands beneath some rolling scars, while excision may remove a selected deep scar and close the resulting wound. Each approach creates a controlled injury that must heal.

Broad tethered hollows, narrow deep pits, and sharp-edged depressions do not respond alike. The clinician evaluates movement with skin stretching, depth, location, nearby vessels and nerves, active acne, and any tendency to form raised or dark scars.

Surgical release may lift a tethered base but may not correct surface roughness or colour. For that reason, resurfacing, microneedling, laser treatment, or carefully selected filler may be discussed at another stage rather than performed as a substitute for structural release.

Give the practitioner a full list of medicines, supplements, allergies, previous procedures, and unusual bleeding or scarring. Mention current skin infection, cold sores near the area, pregnancy, and ongoing acne because these details can alter timing or technique.

Follow wound-care instructions, keep hands away from treated sites, and use only recommended cleansers and dressings. Swelling, bruising, tenderness, temporary colour change, and a new fine scar can occur depending on the procedure; the clinician should explain the expected course and contact plan beforehand.

Contact the treating clinic for worsening pain, expanding redness, pus, fever, persistent bleeding, an opening wound, or swelling that is increasing rather than settling. Sudden pale, dusky, or blistering skin also requires prompt assessment.

Seek urgent help for breathing difficulty, facial swelling suggesting a severe allergic reaction, visual disturbance, or severe pain out of proportion to the treated area. Use the emergency instructions provided by the clinic rather than waiting for a routine follow-up.

Ask which scars the procedure targets, what visible change is realistic, and whether it will leave a different scar. Clarify anaesthesia, recovery, pigment and bleeding risks, practitioner training, alternatives, likely need for staged care, and who will manage an unexpected problem.
