Asteatotic eczema of the ear: care and warning signs

This form of eczema develops when skin loses moisture and protective oils. On the visible ear or the crease behind it, the surface may look rough, flaky or crossed by fine cracks. Itching can lead to rubbing, which further disrupts the skin barrier. Similar changes may also occur on other dry areas of the body. Ear discomfort is not automatically eczema: irritation from hair products, headphones or earrings, contact allergy, infection and other skin conditions can look similar.

Wash the visible outer ear gently with lukewarm water, then pat it dry. Avoid fragranced cleansers, vigorous scrubbing and scratching. A plain, fragrance-free emollient may be applied in a thin layer to affected external skin if the product is suitable for that site. Do not push moisturiser, cotton buds or other objects into the ear canal. Keep potential irritants such as hair dye, sprays and fragranced products away from sore skin. If inflammation persists, a clinician can decide whether a topical corticosteroid is appropriate and specify where and how to use it.

Arrange an assessment if symptoms extend into the ear canal, keep returning, or do not improve with gentle external skin care. Seek prompt care for increasing pain, marked swelling, warmth, pus, an unpleasant smell, fever, sudden hearing change, dizziness or spreading redness. Cracks that bleed repeatedly or produce yellow crust also need review because damaged skin can become infected. A clinician can examine the canal and eardrum before recommending ear preparations.

Ask whether the changes are confined to the outer ear or involve the canal, which moisturiser is suitable for this location, and whether any current hair or skin product may be contributing. If medicine is prescribed, confirm the exact skin area, amount and duration of use. Mention discharge, hearing changes, recent ear treatments, allergies and any pattern linked to jewellery or devices.
