Understanding cholera
Cholera develops after swallowing food or water contaminated with toxigenic Vibrio cholerae bacteria. The toxin prompts the intestine to release large amounts of fluid and salts. Illness may be mild, but cholera can also present with abrupt, profuse watery diarrhoea, vomiting, thirst, weakness, muscle cramps, and rapidly worsening dehydration.
It is not spread by ordinary casual contact in the way a respiratory virus is. Risk is linked mainly to unsafe water, contaminated food, and inadequate sanitation, including during travel or outbreaks.
Immediate steps
Begin a properly prepared oral rehydration solution if cholera is suspected and the person can drink, while arranging medical assessment. Mix packaged solution exactly as instructed using safe water; drinks with an unsuitable balance of sugar and salt do not replace it reliably. Continue breastfeeding an affected infant.
Use safe water for drinking, preparing food, making ice, and cleaning teeth. Wash hands with soap after using the toilet and before handling food. A clinician may use antibiotics in selected cases, but they do not replace rehydration and should not be self-prescribed.
When care is urgent
Seek emergency care for repeated profuse watery stools, inability to keep fluids down, very little urine, marked sleepiness, confusion, fainting, sunken eyes, a very dry mouth, cold skin, or severe weakness. These signs can indicate substantial dehydration and the need for intravenous fluids.
Do not wait for laboratory confirmation before starting oral rehydration when it is safe to drink. Children, older adults, pregnant people, and anyone already frail can deteriorate quickly, so arrange assessment early when significant diarrhoea follows possible exposure.
