Pain between the thumb and index finger

Short answer
Pain between the thumb and index finger may arise from strained muscles, irritated tendons, a joint problem, nerve irritation or an injury.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common trigger
- Forceful or repeated pinching and gripping
- Track
- Exact location, triggering movement and altered sensation
- Urgent sign
- A cold, discoloured or newly numb finger
What may be causing the pain?
Location and triggers help narrow the possibilities. Pain in the web space after gripping differs from soreness at the thumb-side wrist, and numbness or tingling suggests a different process from tenderness caused by direct pressure.
Clues from movement and location
Small muscles between the thumb and index finger help pinch and grip, so forceful or repeated hand use can irritate this area. Tendons controlling the thumb pass closer to the wrist. De Quervain's tenosynovitis commonly presents with pain on the thumb side of the wrist that can travel toward the thumb, particularly during gripping or lifting.
Pain after a fall, sudden sideways force to the thumb or a direct blow may involve a ligament, bone or joint. Pins and needles, burning or reduced feeling may indicate nerve irritation rather than a simple muscle strain.
Practical care for a sore hand
Pause forceful pinching, jar opening, heavy lifting or tool use that reproduces the pain. Keep the fingers and thumb moving gently within a comfortable range so they do not become stiff. A wrapped cool pack can be used briefly after a recent strain; check the skin and stop if it becomes painfully cold or numb.
Avoid repeatedly pressing the tender area to test it. Note whether writing, phone use, wrist movement or gripping is the main trigger. If a splint is considered, correct fit matters because excessive tightness can increase swelling, pressure or altered sensation.
When the hand needs medical attention
Get prompt assessment after an injury if the thumb looks deformed, feels unstable, cannot move normally or cannot hold objects. Urgent care is also appropriate if a finger becomes pale, blue or cold, or if sensation suddenly decreases. Increasing redness, warmth, swelling, drainage, fever, an animal or human bite, or a deep wound needs timely medical attention.
Arrange a non-urgent review when pain persists, repeatedly returns, weakens pinch strength or interferes with sleep and daily tasks.
What to ask during an examination
Ask whether the painful structure appears to be a muscle, tendon, ligament, joint or nerve, and which movements should be limited temporarily. If symptoms followed trauma, ask whether imaging is needed. Clarify which hand exercises are suitable and how to return to work, sport or hobbies without repeatedly provoking the area.

Questions people ask
Could this pain be De Quervain's tenosynovitis?
De Quervain's tenosynovitis often presents with pain at the thumb side of the wrist that may extend toward the thumb. Pain centred only in the web space can have other causes, so location and examination matter.
Should I massage the painful area?
Gentle touch may feel comfortable for a minor strain, but forceful massage can aggravate an acutely injured or swollen structure. Avoid it after significant trauma or when there is marked swelling, bruising or altered sensation.
