What should you know about appendicitis vomiting blood?

Fresh blood may look red, while partly digested blood may resemble dark coffee grounds. Either appearance can signal bleeding in the upper digestive tract and should not be watched at home.

Blood in vomit can arise from the oesophagus, stomach or first part of the small bowel. Possible explanations include an ulcer, inflammation, a tear after forceful retching or bleeding from enlarged veins. Swallowed blood from a nosebleed can sometimes appear in vomit, but that distinction still needs clinical assessment.

Appendicitis more often presents with abdominal pain, loss of appetite, nausea or vomiting without blood. A person can have more than one problem at once, so a previous suggestion of appendicitis should not be used to explain away blood.

Call local emergency services or go to an emergency department now. Do not drive yourself if you feel faint, weak or confused. Avoid eating or drinking while waiting for emergency advice, and do not take aspirin or anti-inflammatory painkillers unless an emergency clinician specifically directs you.

Tell responders about the blood's colour and approximate amount, abdominal pain, black stools, recent retching and all medicines. Bring medication packaging or a current list, especially for blood thinners, painkillers and cancer treatment.

Every episode of vomiting blood needs urgent assessment. An ambulance is particularly important if bleeding continues, the amount is substantial, or there is fainting, clammy skin, rapid breathing, severe abdominal or chest pain, confusion, black tar-like stool or difficulty staying awake.

Ask where the bleeding is thought to have started, which tests can locate it and whether any medicine should be paused or changed. Clarify whether the abdominal pain and bleeding share a cause, how recurrence will be prevented and which symptoms mean returning immediately.
