They are usually shallow, round or oval sores inside the mouth with a pale centre and inflamed edge. In someone with established Crohn's disease, recurring ulcers may reflect inflammation, nutritional deficiency, medicine effects, or an unrelated common mouth-ulcer tendency.

Crohn's disease can affect the digestive tract from the mouth onward, and oral changes may appear before, during, or apart from noticeable bowel symptoms. Aphthous ulcers are one possible oral presentation. Other findings, such as persistent lip swelling, deep linear ulcers, or a cobblestone-like lining of the mouth, need clinical or dental examination.

Many issues can cause similar sores, including minor injury, stress, iron or vitamin deficiency, coeliac disease, infections, and some medicines. A clinician uses the wider pattern—such as abdominal pain, persistent diarrhoea, weight change, growth concerns, fever, or inflammation elsewhere—to decide whether bowel investigation is warranted.

Choose soft foods and cool drinks while the mouth is sore, avoid foods that sting, use a soft toothbrush, and maintain gentle oral hygiene. A pharmacist or clinician can advise on protective pastes, mouth rinses, or topical treatments. Do not place aspirin directly on an ulcer because it can injure the tissue.

Contact your Crohn's care team if ulcers recur with worsening bowel symptoms or appear after a medicine change. Assessment may include examination, a medication review, and blood tests for inflammation, anaemia, iron, folate, or vitamin status. Treatment should address both pain and any identified underlying problem.

Arrange an appointment for an ulcer that persists, repeatedly returns, is unusually large, or occurs with diarrhoea, abdominal pain, fever, weight loss, or swollen lips. Seek urgent care if swelling affects breathing or swallowing, you cannot drink enough, you show signs of dehydration, pass substantial blood, have black stools, or develop severe abdominal pain with a rigid or swollen abdomen.

Ask whether the ulcer pattern could relate to Crohn's activity, a deficiency, or your medicine; whether blood or stool tests are appropriate; and which topical treatment is suitable. If Crohn's disease has not been diagnosed, describe bowel symptoms and family history rather than assuming the mouth lesion identifies the cause.
