What should you know about an AO intramedullary nail?

The nail sits in the marrow canal and is commonly secured with screws above and below the break. Its position allows it to share load with the bone and limit unwanted bending or rotation.

“AO” usually refers to fracture-care principles and implant systems associated with the AO approach. The exact nail shape, diameter, locking pattern, and insertion route depend on the injured bone and fracture pattern. A nail may be used in the femur, tibia, or another suitable long bone; it is not the same as a plate attached to the outside of a bone.

The implant provides internal stability, but healing still depends on the bone forming new tissue across the fracture. Whether the nail remains after healing or is later removed is an individual decision rather than an automatic part of treatment.

Follow the surgeon’s specific instructions for weight-bearing because the safe amount depends on fracture stability, bone quality, and the fixation used. Keep the wound clean as directed, take prescribed medicines correctly, and attend imaging appointments so the team can check alignment and healing.

Physiotherapy often focuses on joint movement, swelling control, muscle activation, walking aids, and a gradual return to daily tasks. Do not advance from crutches or increase impact activity solely because pain has eased; confirm that progression with the treating team.

Contact the surgical team promptly for increasing wound redness, drainage, fever, worsening pain, new numbness, or a foot or hand that becomes unusually pale or cold. Sudden calf swelling, chest pain, or difficulty breathing requires urgent medical assessment.

A new fall, a change in limb shape, loss of previously regained function, or a sudden inability to bear the permitted weight also warrants review because the fracture or hardware may need examination.

Ask which bone and fracture pattern the nail is treating, whether locking screws were used, how much weight you may place through the limb, and which movements are restricted. It is also useful to clarify the wound-care plan, expected imaging schedule, physiotherapy goals, and circumstances in which implant removal might be considered.
