Are anxiety and ulcerative colitis linked?
Anxiety may alter gut movement, appetite and the perception of pain. However, blood in the stool, persistent diarrhoea or worsening abdominal symptoms should not be dismissed as stress.
Understanding the gut–mind connection
Ulcerative colitis involves inflammation of the large bowel. Anxiety can coexist with active disease or remain troublesome when bowel inflammation is controlled. Fear of urgency, accidents or limited toilet access may lead someone to avoid travel, meals or social situations.

Continue prescribed ulcerative colitis medicine unless your treating clinician advises a change. Keep notes on bowel movements, visible blood, pain, food intake, sleep, stress and missed doses; this can reveal patterns without assuming that stress is the sole trigger.
Discuss persistent worry, panic, low mood or avoidance with your care team. Talking therapy, relaxation skills and a practical flare plan can help. Before restricting foods, seek advice so that symptom control does not lead to poor nutrition or dehydration.
Warning signs and urgent care
Seek urgent assessment for severe or rapidly worsening abdominal pain, heavy or continuous rectal bleeding, fainting, confusion, a swollen abdomen, inability to keep fluids down, or signs of marked dehydration. Fever with frequent bloody diarrhoea also needs prompt medical attention.
Contact your inflammatory bowel disease team when symptoms differ from your usual pattern, prescribed treatment is not controlling them, or side effects make adherence difficult. Get immediate mental health support if you feel unsafe or might harm yourself.
