Skin rash around the anus

Gentle cleaning, careful drying, and stopping newly introduced fragranced products can help protect the skin while you arrange assessment for a persistent, painful, spreading, or recurrent rash.

Frequent wiping, stool leakage, soaps, wet wipes, and fragranced products may provoke contact dermatitis, which can present with burning, itching, redness, or scaling where skin met the irritant. Intertrigo can present as sore, moist inflammation within a skin fold, sometimes complicated by yeast or bacteria.

Psoriasis can present around the anus as a well-defined inflamed patch with less scale than on drier body sites. Ringworm can present with an enlarging scaly border, and scabies can present with intense itch that also affects other characteristic body areas. Symptoms overlap, so treatment based only on a photograph or guess may make the problem worse.

Rinse gently with lukewarm water after bowel movements when practical, then pat dry. Replace perfumed wipes and harsh soap with an unperfumed cleanser if one is needed. Loose, breathable underwear and prompt changes after sweating help keep the fold from remaining damp.

Avoid scratching and keep fingernails short. Do not combine antifungal, antibiotic, steroid, and numbing creams without a diagnosis; steroid use can alter the appearance of an infection. Bring a list of skin products and medicines to an appointment, including anything started shortly before the rash.

Seek urgent medical care for fever with rapidly spreading redness, severe pain, substantial swelling, pus, blisters with extensive peeling, or trouble breathing after a new medicine. Arrange a routine examination for a rash that persists, repeatedly returns, bleeds, disrupts sleep, extends elsewhere, or accompanies ongoing bowel leakage.

Ask whether the distribution suggests irritation, an inflammatory disorder, or infection, and whether a scraping, swab, or examination elsewhere on the skin would help. Clarify exactly where and how long to use any prescribed cream, because anal skin is sensitive and different treatments can look similar in their packaging.
