Choice and dose depend on kidney function, urine protein, other conditions, potassium levels and current medicines. Monitoring helps the prescriber balance benefit with dizziness, electrolyte changes and changes in kidney function.

Several medicine groups lower blood pressure in different ways. ACE inhibitors or angiotensin receptor blockers may be chosen when kidney protection is a treatment goal, particularly when protein is present in urine. Diuretics, calcium-channel blockers, beta blockers or other medicines may be added or used instead according to the wider clinical picture.

A small change in kidney blood-test results can occur after starting or adjusting certain medicines. Clinicians interpret this alongside potassium, hydration, symptoms and the underlying kidney condition rather than judging one result in isolation.

Take each medicine as prescribed and attend planned blood pressure, kidney function and electrolyte checks. Keep a current medicine list that includes supplements, herbal products and non-prescription painkillers. Some anti-inflammatory painkillers can strain the kidneys, especially during dehydration or alongside certain blood pressure medicines.

Do not stop treatment because a home reading is lower than usual unless a clinician has given you a specific threshold and plan. During vomiting, diarrhoea, fever or poor fluid intake, seek personalised sick-day advice because temporary changes may be needed for some medicines.

Seek emergency care for chest pressure, severe breathing difficulty, collapse, new facial droop, arm weakness, speech trouble or sudden severe confusion. These symptoms require urgent assessment regardless of a recent blood pressure reading.

Contact your clinical team promptly for fainting, persistent severe dizziness, very little urine, worsening swelling, repeated vomiting, new muscle weakness or an irregular heartbeat. Swelling of the lips, tongue or throat after a medicine requires immediate emergency help.
