Antihistamines in anaphylaxis treatment

They act too slowly and do not reliably treat dangerous airway swelling, breathing difficulty or a sudden fall in blood pressure. Do not wait to see whether an antihistamine works before following an anaphylaxis emergency plan.

Anaphylaxis can affect the airway, breathing and circulation. Adrenaline is the priority medicine because it addresses these life-threatening effects. An antihistamine may later be considered by a clinician for persistent hives or itching, once emergency treatment is underway.

Feeling better after an antihistamine does not show that the reaction has ended. Serious features can continue or return, so medical observation and the instructions of the emergency team still matter.

Use the prescribed adrenaline auto-injector immediately if the person's action plan says to do so, then call UAE emergency services. Help the person lie down; if breathing is difficult, they may sit with legs extended. Do not let them stand or walk suddenly.

If symptoms persist and a second auto-injector is available, follow the timing in the person's emergency plan or the dispatcher's instructions. Note when each dose was given and hand used devices to the ambulance crew. Antihistamines must not delay any of these actions.

Treat throat or tongue swelling, noisy or difficult breathing, wheezing, a hoarse voice, faintness, collapse, confusion, or rapidly worsening symptoms after possible allergen exposure as an emergency. Call emergency services even if adrenaline improves the symptoms.

A widespread rash alone may have several causes, but rash accompanied by breathing, throat or circulation symptoms needs immediate escalation. Someone who has received adrenaline for suspected anaphylaxis needs assessment by emergency clinicians.
