Antihistamines for poison ivy rash

Poison ivy causes allergic contact dermatitis after plant oil touches the skin. This delayed immune reaction is not driven mainly by histamine, so an antihistamine has a limited role compared with skin-directed treatment.

The rash can become red, swollen, blistered, and intensely itchy in the areas that contacted plant oil. Blister fluid does not spread poison ivy. New patches can seem to appear because different areas received different amounts of oil or because oil remained on clothing, tools, shoes, or pet fur.

A sedating oral antihistamine may make nighttime itching easier to tolerate by causing drowsiness. Antihistamine creams are generally poor choices because they can irritate or sensitise the skin and may add another rash.

As soon as possible after contact, wash exposed skin gently with soap and water and clean beneath the nails. Launder contaminated clothing separately and wash objects that may hold plant oil while wearing protective gloves. Cool wet compresses and a suitable anti-inflammatory skin treatment can soothe the reaction; a pharmacist can help choose an option for the affected area.

If you take an oral antihistamine, follow the label and avoid alcohol, driving, or hazardous work when sleepy. Do not combine it with a sleep aid or another antihistamine without professional advice, and check before use in a young child, pregnancy, breastfeeding, glaucoma, or urinary difficulty.

Seek urgent help if there is trouble breathing, facial or throat swelling, faintness, or a severe reaction after smoke from burning plants. Prompt clinical advice is also appropriate for extensive blistering, rash around the eyes or genitals, rapidly increasing pain, pus, spreading warmth, fever, or symptoms that keep worsening. A clinician may prescribe stronger anti-inflammatory treatment when the reaction is severe or involves sensitive sites.

Ask whether the appearance and exposure history fit allergic contact dermatitis, which topical product is safe for the body area involved, and whether an oral medicine is needed. Describe any spreading redness, discharge, fever, eye involvement, or sleep loss.
