Antihistamines and blood pressure

The active ingredients matter more than the product’s front-label description. People with high blood pressure, heart disease or several medicines should check the full label with a pharmacist or clinician.

Antihistamines block histamine signals and can relieve a runny nose, itchy eyes and sneezing. Some older, sedating antihistamines may cause drowsiness, blurred vision or dizziness. Dizziness can complicate blood pressure symptoms and increase the chance of falls, particularly alongside alcohol, sleeping tablets or other sedating medicines.

Decongestants narrow blood vessels in the nose. Oral ingredients such as pseudoephedrine can also increase blood pressure or cause a fast, pounding heartbeat. They are often packaged with an antihistamine, so a product marketed for allergy, sinus or cold relief may contain both types of medicine.

Read the active-ingredient panel and look for a decongestant rather than relying on the brand name. Ask a pharmacist for a single-ingredient option suited to your symptoms and health history. Mention high blood pressure, glaucoma, prostate or urinary problems, pregnancy, and all prescription and non-prescription medicines.

Continue prescribed blood pressure treatment unless the prescriber tells you to change it. Do not double an allergy dose or take two cold-and-allergy products together, because they may contain the same ingredients. Saline nasal rinses and reducing exposure to a known allergen may help a blocked nose without adding an oral decongestant.

Contact a clinician or pharmacist if blood pressure readings rise after starting a product, palpitations persist, dizziness interferes with walking, or allergy symptoms repeatedly require medicine. Bring the packet or a photograph of its ingredient list so the exact formulation can be checked.

Seek urgent care for chest pain, severe breathlessness, fainting, sudden weakness or numbness, trouble speaking, a severe unusual headache, or swelling of the tongue or throat. These symptoms need prompt assessment regardless of whether an antihistamine was recently taken.

Ask which single ingredient fits your symptoms, whether it interacts with your blood pressure medicines, if it may cause drowsiness, how long to use it, and whether a nasal spray or non-drug measure would be more suitable.
