Can antibiotics cause insomnia?

Sleep changes can include difficulty falling asleep, waking often, vivid dreams, or feeling restless at night. The timing matters: symptoms that begin soon after starting a medicine and improve after the course ends should be discussed with the prescriber, but do not stop treatment on your own.

What the sleep change may mean

An antibiotic can affect people differently. Some medicines may make a person feel wired or unsettled, while nausea, pain, fever, coughing, or diarrhoea from the illness can keep sleep away. Caffeine, decongestants, steroid medicines, and other treatments taken at the same time may also contribute.

Insomnia is not a reason to assume the antibiotic is harmful or that the infection is worsening. A clinician or pharmacist can review the exact antibiotic, dose, timing, other medicines, and your symptoms to identify a safer adjustment when appropriate.

What you can do now

Take the antibiotic exactly as prescribed, including any instructions about food and timing. Do not skip doses, double a dose, share tablets, or use leftover antibiotics. If the label allows flexibility and sleep disruption follows an evening dose, ask a pharmacist whether another time is suitable before changing it.

Keep a brief record of when you take each medicine, when sleep problems start, and any accompanying symptoms. Limit late-day caffeine and nicotine, keep the bedroom cool and dark, and use a calm wind-down routine. Avoid starting sleeping tablets, antihistamines, or herbal products without checking for interactions.

Contact the prescriber or pharmacist if insomnia is severe, lasts beyond a short period, affects driving or work, or is accompanied by marked agitation, confusion, or unusual thoughts. They may decide whether to change the medicine or investigate another cause.

Seek urgent medical help for trouble breathing, swelling of the lips or face, widespread blistering or peeling skin, fainting, or a rapidly spreading rash after an antibiotic. These can signal a serious reaction.

Get prompt assessment for severe confusion, hallucinations, extreme restlessness, a seizure, chest pain, or thoughts of harming yourself. If vomiting or diarrhoea prevents you from keeping the medicine or fluids down, contact a clinician for advice about the infection and treatment.
