Antibiotics for a UTI during pregnancy

Pregnancy affects which antibiotics are appropriate and makes timely treatment particularly important because infection can move from the bladder towards the kidneys. Burning when passing urine, needing to urinate more urgently, lower abdominal discomfort, or cloudy or strong-smelling urine can occur with a UTI, although pregnancy itself can also increase urinary frequency.

A urine culture can identify bacteria and show which medicines are likely to work. Sometimes bacteria are found without typical urinary symptoms; the maternity clinician will decide whether treatment is required and whether another sample is needed after the course.

Use the prescribed dose for the full instructed course, even if discomfort settles earlier. Before the first dose, make sure the prescriber knows how far the pregnancy has progressed, any antibiotic allergies, kidney problems, recent urine-culture results, and every prescription, non-prescription medicine, or supplement you take.

Do not take leftover antibiotics or share medication. Drink enough fluid to avoid dehydration unless your maternity team has restricted fluids for another reason. If you vomit after a dose, miss a dose, or develop troublesome side effects, ask a pharmacist or the prescriber how to proceed rather than changing the schedule yourself.

Seek urgent medical assessment for fever, chills, pain in the back or side below the ribs, vomiting, or feeling severely unwell. These can accompany a kidney infection and may require hospital-based treatment. Also contact your maternity service urgently for vaginal bleeding, fluid leaking from the vagina, regular painful tightenings, or reduced fetal movement when movements are normally felt.

Call emergency services for breathing difficulty, collapse, confusion, or swelling of the face, lips, tongue, or throat after medicine. If urinary symptoms persist or return after treatment, arrange a prompt review and repeat urine testing rather than starting another course independently.

Confirm why the selected antibiotic suits your stage of pregnancy, when the culture result will be available, and whether it could alter treatment. Ask whether you need a repeat urine sample, what improvement to expect, and exactly whom to contact if symptoms continue outside clinic hours.
