Antibiotics for tonsillitis

Short answer
Antibiotics may be offered for tonsillitis when bacterial infection is likely or confirmed, but they do not treat the viral infections that also inflame the tonsils.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Possible causes
- Both viruses and bacteria can inflame the tonsils
- Antibiotic use
- Reserved for selected bacterial illness
- Immediate priority
- Breathing difficulty or inability to swallow saliva needs urgent help
The short answer
A clinician considers the pattern of illness, examines the throat and neck, and may use a swab or other test in selected situations. This avoids unnecessary treatment and helps identify complications that need a different response.
How the treatment decision is made
Tonsillitis means inflammation of the tonsils. It can cause painful swallowing, fever, swollen neck glands, bad breath, and red or coated tonsils. These findings can occur with viral and bacterial causes, so visible white patches alone do not establish that antibiotics are needed.
The selected antibiotic depends on allergy history, previous reactions, pregnancy, other health conditions, and medicines that could interact. Taking antibiotics unnecessarily can cause side effects and encourages bacteria to become harder to treat in future.
Making swallowing easier
Sip fluids frequently; cool drinks, ice lollies, soups, or soft foods may be easier to swallow. Rest and avoid smoke or other throat irritants. Ask a pharmacist about age-appropriate pain relief and check suitability if you are pregnant, have stomach, kidney, or liver problems, or take other medicines.
Take a prescribed antibiotic at the instructed times and for the stated course. Do not share doses or use an old supply. If swallowing tablets is difficult, ask about a suitable formulation instead of crushing medicine without advice.
Warning signs around the throat and airway
Call for urgent help if breathing is difficult, saliva cannot be swallowed, drooling develops, or the person becomes confused or unusually drowsy. Urgent assessment is also needed for a muffled voice, marked one-sided swelling, severe pain on one side, neck stiffness or swelling, or difficulty opening the mouth.
Seek clinical advice if drinking becomes difficult, urine is reduced or dark, symptoms worsen or return after improvement, a new rash develops, or prescribed treatment is not helping. Reduced immunity and serious chronic illness lower the threshold for an earlier review.
Questions that can shape your care
Ask what points suggest a bacterial or viral cause, whether a swab would be useful, how to maintain fluid intake, and when improvement should prompt no further action versus reassessment. Report past reactions to penicillin or other antibiotics precisely, including the symptoms and when they occurred.

Questions people ask
Do white spots on the tonsils prove the infection is bacterial?
No. Coating or spots may appear with different causes. The clinician combines symptoms, examination findings, and any appropriate test before deciding about antibiotics.
Can tonsillitis spread to other people?
The viruses and bacteria that cause tonsillitis can spread. Handwashing, covering coughs and sneezes, and not sharing cups or utensils can reduce transmission.
What should I do if an antibiotic causes a rash?
Contact a clinician promptly for advice. Seek emergency help if the rash occurs with facial or tongue swelling, breathing difficulty, faintness, or rapidly worsening symptoms.
