Mites and bacterial infection need different treatment

Scabies is caused by mites that burrow into the upper skin, often producing intense itching and a rash. It requires a scabicide, which may be applied to the skin or taken by mouth depending on clinical circumstances. An antibiotic cannot remove the mites or prevent untreated scabies from spreading.

Repeated scratching can break the skin and allow bacteria to enter. Increasing pain, warmth, swelling, pus, crusting, or redness spreading beyond the original rash may suggest secondary infection. A clinician can distinguish this from ongoing irritation or an expected itch after mite treatment.

Treat the infestation completely

Use the scabies medicine over every body area specified in its instructions, leave it on for the directed period, and repeat it only when advised. People who live together and other close physical contacts often need coordinated treatment even if they are not yet itchy, because symptoms can take time to appear.

Wash or isolate recently used clothing, towels, and bedding according to the treatment instructions. Keep nails short, avoid sharing fabrics, and try not to scratch. If an antibiotic is also prescribed, take it for the bacterial infection exactly as directed; it does not replace the scabies medicine.

When the skin needs prompt assessment

Seek same-day medical care if redness is expanding, the skin is increasingly hot or painful, pus is draining, fever develops, or you feel generally unwell. Urgent help is needed for rapidly worsening swelling, red streaks, confusion, or breathing difficulty. Babies, people with weakened immunity, and anyone with widespread thick crusting should be assessed rather than relying on over-the-counter treatment alone.

Confirm which areas need the scabies medicine, whether household members should be treated together, and how fabrics should be handled. Ask whether visible changes represent bacterial infection and, if an antibiotic is prescribed, which side effects require attention. Can itching continue after scabies treatment?

Yes. Skin inflammation can persist after the mites have been treated, so itching alone does not prove that an antibiotic or another scabies dose is needed.

Should close contacts wait for a rash?

Often they should be treated at the same time, but the exact contact plan depends on the diagnosis and medicine. Coordinated treatment reduces reinfestation.
