Antibiotics for pneumonia

Short answer
Antibiotics are used for bacterial pneumonia, while viral pneumonia may need different treatment and supportive care.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Antibiotics treat
- Bacterial pneumonia, not pneumonia caused only by a virus
- Choice depends on
- Clinical setting, severity, health history and allergies
- Recovery
- Cough and tiredness may improve more slowly than fever
The short answer
A clinician chooses treatment after considering symptoms, examination findings, illness severity and personal health risks. Some people can recover at home, while others need hospital monitoring, oxygen or medicine given through a vein.
How treatment is chosen
Pneumonia is an infection of lung tissue. Bacteria and viruses can produce overlapping symptoms, including cough, fever, breathlessness and chest discomfort, so symptoms alone may not reveal the cause. Examination, oxygen measurement and sometimes chest imaging or laboratory tests help guide care.
The prescription can differ according to where the infection developed, recent antibiotic use, allergies, pregnancy, kidney or liver function and other illnesses. A change of antibiotic may be considered if the clinical response is poor or test results identify a different organism.
Managing treatment at home
Start a prescribed antibiotic as directed, space doses according to the label and finish the course unless a clinician advises otherwise. Check what to do about a missed dose rather than doubling it. Avoid taking leftover tablets because the drug, dose and course may be wrong.
Drink enough fluid, rest and avoid smoking. Fever may settle before cough and tiredness do, but the overall trend should be toward recovery. Arrange review if symptoms fail to improve as expected, return after improving, or side effects interfere with taking the medicine.
When breathing needs urgent help
Get emergency help for severe difficulty breathing, blue or grey lips, new confusion, collapse, extreme drowsiness, or severe chest pain. A child who is struggling for breath, grunting, unusually floppy or hard to wake also needs immediate assessment.
Seek prompt medical advice for worsening breathlessness, inability to drink, repeated vomiting, reduced urination, coughing up blood or a persistent high fever. Older adults and people with significant heart, lung or immune conditions should have a low threshold for reassessment when their condition declines.
Making the plan clear
Ask whether the pneumonia is thought to be bacterial, what improvement to watch for, when follow-up is needed and which adverse effects require a call. Also confirm whether regular medicines should continue and whether any chest imaging needs to be repeated after recovery.

Questions people ask
Can I choose an antibiotic that worked for a previous chest infection?
No. The likely cause and suitable dose may differ, and some antibiotics interact with regular medicines or need adjustment for other conditions.
What if I vomit after taking a dose?
Contact a pharmacist or prescriber for instructions. Whether another dose is appropriate depends on the medicine and when vomiting occurred.
Why might pneumonia need hospital care?
Hospital care allows close monitoring and support such as oxygen, fluids or intravenous medicine when breathing, hydration or general condition is concerning.
