The short answer

Pleurisy describes inflammation of the thin layers around the lungs, not a single infection. Treatment therefore depends on finding the cause; antibiotics do not treat viral illness, a blood clot or autoimmune inflammation.

What pleuritic pain can mean

Pleuritic pain is often sharp and becomes worse with a deep breath, cough or movement. A clinician may examine breathing, oxygen level and signs of infection, then decide whether chest imaging, blood tests or other investigations are needed.

When bacterial pneumonia or another bacterial chest infection is identified, an antibiotic may form part of treatment. Pain relief and treatment of the underlying inflammation may also be needed. The antibiotic choice is guided by the suspected infection, illness severity, allergies and other health conditions.

What you can do now

New pain linked to breathing should be medically assessed rather than treated with leftover antibiotics. Until reviewed, note when the pain began, whether it came on suddenly, and whether you have cough, fever, breathlessness, leg swelling or recent immobility.

If an antibiotic is prescribed, use it at the stated dose and intervals and follow instructions about food. Contact the prescriber if vomiting prevents doses from staying down or side effects make the course difficult to continue. Rest and avoid smoking while recovering.

Chest pain red flags

Call emergency services for severe or sudden chest pain, marked breathing difficulty, blue or grey lips, coughing up more than small streaks of blood, fainting, confusion, or pain accompanied by sweating or pressure spreading to the arm, jaw or back.

Seek prompt assessment for fever with worsening breathlessness, persistent pain on breathing, a fast or irregular heartbeat, or one-sided leg pain and swelling. These features require evaluation of the cause and should not be used to decide on antibiotics at home.

Questions after diagnosis

Ask what is thought to be inflaming the pleura, whether tests or follow-up imaging are needed, how to control pain safely, and which change in breathing should trigger reassessment. If antibiotics are prescribed, clarify what to do after a missed dose and when improvement should be checked.

No. Several infectious and non-infectious problems can cause this pain pattern, so the cause needs assessment before an antibiotic is considered.

Can pain improve before the infection is fully treated?

Symptoms can change at a different pace from the underlying illness. Continue prescribed treatment and seek review if breathing or general condition worsens.

Is pleurisy contagious?

The inflammation itself is not passed between people. An infection causing it may be transmissible, depending on the particular organism.
