Antibiotics for mono: when they help and when they do not

The key distinction
Using an antibiotic without a bacterial target will not shorten the viral illness and can cause side effects. Certain penicillin-family antibiotics are particularly associated with a widespread rash when given during mono. That rash needs medical review; it does not by itself prove a lasting penicillin allergy.
Why symptoms do not settle with antibiotics
Mono, also called infectious mononucleosis, commonly causes severe sore throat, swollen neck glands, fever, and pronounced fatigue. The spleen or liver can become enlarged. Recovery relies mainly on the immune system, so treatment focuses on fluids, rest, and suitable relief for fever and throat pain.
A throat that looks inflamed or has white patches can occur with mono and does not identify a bacterial cause on appearance alone. A clinician may examine you and test when strep throat or another infection is suspected. If bacteria are confirmed or strongly suspected, the prescription should target that separate diagnosis while taking the mono history into account.
Managing recovery
Drink regularly; cool liquids and soft foods may be easier with a painful throat. Use pain or fever medicine only if it is suitable for your age, health conditions, and other treatments. Do not share drinks, utensils, toothbrushes, or lip products because saliva can spread the virus.
Avoid contact sports, heavy lifting, and activities that could strike the abdomen until a clinician says they are safe, because an enlarged spleen is vulnerable to injury. Return to work, school, and exercise gradually as energy improves. If an antibiotic was prescribed, confirm what bacterial infection it is intended to treat and report any rash promptly.
Symptoms that need prompt help
Seek emergency care for sudden or severe pain in the upper left abdomen, pain that reaches the left shoulder, fainting, marked dizziness, or collapse. These can indicate a spleen complication. Breathing difficulty, inability to swallow saliva, noisy breathing, or rapidly increasing throat swelling also requires urgent assessment.
Arrange medical review if you cannot maintain fluids, urine becomes very dark, skin or eyes look yellow, fever returns after improving, or fatigue is worsening rather than gradually easing. After taking medicine, facial swelling, wheezing, or a blistering or peeling rash is an emergency.
