Antibiotics for kidney stones

A stone can obstruct urine flow. When infection is present behind an obstruction, urgent hospital treatment may be needed because antibiotics alone may not drain the infected urine. The stone and the infection therefore require separate decisions.

Kidney stones often present with intense pain in the side or back that can move toward the lower abdomen or groin. Blood in urine, nausea and restlessness can occur. Painful urination or urinary urgency may accompany a stone, but these symptoms also prompt a check for infection.

Urine testing can look for signs of bacteria and inflammation, while imaging can identify the stone and possible blockage. An abdominal ultrasound may be suitable in some situations. A urine culture can help the clinician select an antibiotic when infection is found.

Seek a same-day clinical review when stone-like pain occurs with burning urination, cloudy or foul-smelling urine, or feeling feverish. A GP consultation can help arrange urine tests, pain relief and imaging, or direct you to emergency care when obstruction or infection is suspected.

Take a prescribed antibiotic exactly as directed, but return promptly if fever, pain or vomiting worsens. Do not use leftover tablets. Drink according to medical advice; repeatedly forcing large volumes while severe pain or blockage is suspected may increase discomfort, and persistent vomiting can make dehydration worse.

Go to emergency care for flank pain with fever, shaking chills, confusion, marked weakness or repeated vomiting. Difficulty passing urine or producing very little urine also needs urgent assessment, especially when pain is severe or only one functioning kidney is known.

Emergency review is warranted if pain cannot be controlled, fainting occurs, or pregnancy is possible and severe abdominal or flank pain develops. Breathing difficulty or swelling of the face, tongue or throat after an antibiotic may indicate a serious allergic reaction.

Ask whether urine appears infected, whether culture or imaging is needed, and whether the stone is blocking flow. Clarify the plan for pain relief, how the stone will be followed, and which symptoms mean you should go directly to hospital.
