Antibiotics for hand, foot and mouth disease

Short answer

Antibiotics do not treat hand, foot and mouth disease because viruses, rather than bacteria, cause the illness.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Cause
A viral infection
Antibiotics
Do not treat the underlying illness
Main concern
Dehydration from painful mouth sores

The short answer

Care focuses on fluids, comfort and watching for warning signs while the immune system clears the infection. An antibiotic would only be considered if a clinician diagnosed a separate bacterial infection, not for the viral rash or mouth ulcers themselves.

Recognising the illness

Hand, foot and mouth disease commonly causes fever, reduced appetite, painful mouth sores and spots or small blisters on the hands and feet. Spots can also appear around the buttocks, legs or arms. The mouth pain may make young children reluctant to drink.

The viruses spread through respiratory secretions, blister fluid and stool. Good handwashing, careful disposal of nappies, cleaning frequently touched surfaces and avoiding shared cups or cutlery can reduce spread within a household.

Comfort and hydration at home

Offer frequent cool drinks and soft foods that do not sting. Acidic, salty or spicy items may irritate mouth ulcers. Age-appropriate pain or fever medicine may help when used according to its label and a pharmacist's or clinician's advice; aspirin should not be given to children.

Keep blistered skin clean and avoid deliberately opening blisters. Do not use leftover antibiotics. If the diagnosis is uncertain, arrange an assessment because chickenpox, herpes infections and other rashes can look different but still require specific advice.

When medical review is needed

Seek prompt care if the person drinks very little, urinates much less than usual, has a dry mouth, has no tears when crying, becomes unusually sleepy or cannot keep fluids down. Urgent assessment is also appropriate for breathing difficulty, a stiff neck, confusion, a seizure, severe headache, rapidly worsening illness or a rash that does not fade when pressed.

Young infants, people with weakened immunity and pregnant people should contact a clinician for tailored advice after significant exposure or symptoms.

Questions at an appointment

Ask whether the pattern fits hand, foot and mouth disease, which fluids and pain relief suit the person's age and health, and how to recognise dehydration. Clarify when return to nursery, school or work is appropriate under local guidance.

Questions people ask

Do yellow-looking blisters mean antibiotics are needed?

Not by colour alone. A clinician looks for evidence of a separate bacterial skin infection, such as expanding redness, warmth, increasing pain or pus.

Can adults get hand, foot and mouth disease?

Yes. Adults can become infected and may have mild symptoms or a more noticeable sore throat, mouth ulcers and rash.

Should blisters be covered?

Avoid opening them. Keep the skin clean; a light, non-stick covering can protect a blister if friction or leakage is a problem.
