The short answer

Many eczema flares are red, itchy, cracked, or weeping without requiring antibiotics, so treatment decisions rely on the pattern of symptoms and a skin examination.

Eczema flare or bacterial infection

Eczema weakens the skin barrier, and scratching creates openings where bacteria can enter. Increasing pain, warmth, swelling, pus-filled spots, yellow crusting, rapidly spreading redness, or fever can support concern about bacterial infection. Weeping alone does not establish that bacteria are responsible.

A clinician may use a topical antibiotic for a small, localised infection or an oral antibiotic when infection is widespread or accompanied by systemic illness. Repeated or prolonged antibiotic use can encourage resistance and may cause side effects, so it is not routine flare treatment.

Manage infection and eczema together

Continue the eczema plan advised for you, which may include regular emollient and an anti-inflammatory treatment. Apply products with clean hands, avoid scratching where possible, keep nails short, and do not share towels. Ask how to space different creams so each is used correctly.

Use any antibiotic only for the prescribed area and course. Do not apply an old antibiotic cream to a new rash, because the diagnosis may be different. If flares repeatedly look infected, a clinician can review triggers, skin care, treatment technique, and whether a swab would help.

Symptoms that need prompt assessment

Arrange urgent medical review when redness spreads quickly, the skin becomes very painful or swollen, fever or marked unwellness develops, or red streaks extend away from the affected area. Infection close to the eyes also needs prompt assessment.

Clusters of painful blisters, punched-out sores, rapidly worsening eczema, eye discomfort, or unusual sleepiness require urgent evaluation because a viral eczema complication can resemble bacterial infection and needs different medicine. Seek emergency help for breathing difficulty or swelling of the mouth or throat.

Clarify the treatment plan

Ask which findings suggest infection, where each prescribed product belongs, and whether your usual eczema medicines should continue. Find out what response to expect, which adverse effects matter, and when a swab or specialist review would be useful.
