Antibiotics for croup: what parents should know

A clinician may give a corticosteroid to reduce swelling around the voice box and windpipe. A child with more pronounced breathing difficulty may need inhaled treatment and observation. Antibiotics are reserved for a separate suspected bacterial infection, not the barking cough itself.

Croup commonly causes a harsh, seal-like cough, a hoarse voice, and a high-pitched sound when breathing in called stridor. Symptoms may become more noticeable at night or when a child cries because the already narrowed upper airway has to move air faster.

Antibiotics target bacteria and cannot reduce viral inflammation. A clinician still needs to consider other causes when the pattern is unusual, swallowing is difficult, the child drools, illness appears severe, or there may be an inhaled object.

Keep the child calm and sitting upright, since distress can make noisy breathing worse. Offer frequent drinks and check them during the night. Use fever or discomfort medicine only in the age-appropriate form and dose; a pharmacist or clinician can help if you are uncertain.

Do not expose a child to steam from boiling water because burns can occur, and avoid cough medicines unless a clinician specifically recommends one. If croup has been diagnosed, follow the return precautions carefully because breathing can change over a short period.

Call emergency services if the child has blue or grey lips, pauses in breathing, extreme sleepiness, severe agitation, struggles for each breath, or has marked pulling-in of the skin between or below the ribs. Stridor while calm, rather than only while crying, also needs urgent assessment.

Obtain immediate medical help if the child is drooling and cannot swallow, wants to sit forward to breathe, or appears suddenly very unwell. These findings can point to a different airway emergency. A young infant with a barking cough or any child whose symptoms are escalating should be assessed promptly.

Ask whether the breathing noise is stridor, whether a steroid is recommended, and how long its effect should take to appear. Confirm which signs mean returning immediately, how to give fluids safely, and whether recurrent episodes need further evaluation. Mention choking, allergy exposure, drooling, or a previous airway problem.
