Antibiotics for a chalazion: when are they needed?

An eye clinician may prescribe an antibiotic when there are signs of infection in the eyelid or surrounding skin. Antibiotic eye drops should not be started from an old prescription or shared with another person.

A chalazion forms when oil is trapped in a small eyelid gland, creating a firm lump. It may begin with tenderness and later become less painful. Because the main problem is blockage and inflammation, an antibiotic cannot open the gland.

A painful red swelling at the eyelash edge may instead be a stye, while spreading redness, warmth, marked tenderness, or discharge can suggest infection. An examination helps distinguish these problems from conjunctivitis and other eye conditions.

Hold a comfortably warm, clean compress against the closed eyelid several times through the day, then use very gentle lid massage if it does not hurt. Wash your hands before touching the area, pause eye makeup and contact lenses while the lid is irritated, and do not squeeze or pierce the lump.

Arrange an eye assessment if the lump persists, returns in the same place, interferes with vision, or remains troublesome. A clinician can check the eyelid and discuss drainage or an anti-inflammatory treatment when simple care is not enough.

Get urgent medical help for reduced vision, severe eye pain, pain when moving the eye, fever with worsening eyelid swelling, redness spreading around the eye, a bulging eye, or difficulty opening the eye. These features require assessment for a deeper or spreading infection rather than treatment as a routine chalazion.

Seek prompt advice sooner if you have a weakened immune system, recent eye surgery, or an eye injury. A child with rapidly increasing swelling or who appears unwell should also be assessed without delay.

Ask whether the lump is a chalazion, a stye, or an eyelid infection; whether warm compresses are suitable; and which change would justify urgent review. If an antibiotic is prescribed, ask what infection it is treating, how to apply it, and what side effects should make you stop and seek advice.
