Antibiotics for appendix problems

Antibiotics can treat infection linked to an inflamed appendix, but they should only be used after urgent assessment because some appendix problems also need surgery or drainage. The medicine does not remove a blockage or drain a collection of pus, so the hospital team first needs to identify what is happening around the appendix.

People often use "appendix infection" to mean appendicitis, but pain in the same area can come from the bowel, urinary tract or reproductive organs. Examination, blood tests and imaging help distinguish these causes and show whether inflammation is uncomplicated or has led to perforation or an abscess. Antibiotic choice then depends on the suspected bacteria, illness severity, allergies and whether a procedure will control the source.

Arrange urgent assessment for persistent pain on the lower right side of the abdomen, particularly with nausea, vomiting, fever or reduced appetite. Avoid treating yourself with antibiotics saved from another illness; the drug or dose may not cover the relevant bacteria, and partial improvement can delay recognition of a complication. Tell the clinical team about allergies, recent antibiotics, pregnancy or possible pregnancy, and medicines that affect bleeding or immunity.

Call emergency services or go to an emergency department for intense or rapidly worsening abdominal pain, collapse, confusion, breathing difficulty, repeated vomiting, or a rigid and very tender abdomen. Pain spreading across the abdomen, new marked weakness or deterioration after an apparent improvement also needs immediate attention. If you are already taking prescribed antibiotics, worsening symptoms require reassessment rather than an extra or doubled dose.

Ask what the tests show around the appendix, whether there is an abscess or perforation, and whether the aim of antibiotics is preparation for surgery, treatment after a procedure or non-operative care. Confirm how doses are given, how side effects should be reported, when symptoms should begin to settle and where to return if they do not.
