Antibiotics for a skin ulcer: when they are needed

They do not close the ulcer or correct causes such as pressure, poor circulation, nerve damage, or ongoing friction. A clinician should examine the wound before choosing treatment because an open ulcer can contain bacteria without having an active infection.

Increasing pain, warmth, swelling, spreading redness, pus, unpleasant odour, or feeling unwell can point to infection. Redness may look purple, grey, or darker than nearby skin on brown or black skin. The prescriber considers the ulcer's site and depth, recent antibiotics, allergies, circulation, diabetes, and any immune-system condition.

A sample is sometimes taken when the infection is severe, unusual, or not improving. Results can help the clinician adjust the antibiotic, but wound appearance and the person's overall health guide the initial decision.

Take a prescribed antibiotic exactly as directed and do not use leftover tablets or someone else's medicine. Keep scheduled dressing changes, protect the area from pressure or rubbing, and follow the wound team's cleaning instructions. Do not start compression bandaging unless circulation has been assessed and a clinician has recommended it.

Track whether pain, drainage, swelling, and the edge of redness are improving. Even when infection settles, the underlying cause still needs treatment to support healing and reduce recurrence.

Arrange prompt assessment for rapidly spreading redness, worsening pain, new pus, fever, chills, confusion, unusual drowsiness, or red streaks extending from the ulcer. Emergency care is appropriate for breathing difficulty, faintness, severe weakness, or skin that becomes dusky, blistered, numb, or black.

Seek early review if you have diabetes, reduced sensation, poor blood flow, or a weakened immune system, because infection and tissue damage may progress with less obvious warning.

Ask what signs support an infection, what is causing the ulcer, how dressings should be changed, and when improvement should become noticeable. Confirm what to do after a missed dose, which medicine reactions need urgent help, and who will reassess the wound if it does not improve.
