Antibiotics for hot tub folliculitis: what to know

This rash is usually caused by Pseudomonas bacteria in contaminated warm water. Because many mild cases improve after exposure stops, an antibiotic is not automatically helpful. A clinician may consider a prescription when the eruption is extensive, does not begin to settle, is getting worse, or occurs in someone whose immune defences are reduced. The medicine must be chosen for the likely bacterium; leftover antibiotics or a general antibiotic cream may not be suitable.

Hot tub folliculitis produces itchy or tender bumps centred on hair follicles, sometimes with small pus-filled tops. It often appears after time in a hot tub, spa pool, or poorly maintained pool and may be more noticeable beneath swimwear, where contaminated water stayed against the skin. A clinician may reconsider the diagnosis if there are deep lumps, large boils, marked pain, or no clear water exposure.

The appearance can overlap with other follicle infections, irritation, and reactions to swimwear products. That is why the exposure history, distribution, and progression matter more than choosing an antibiotic from the rash alone.

Avoid the suspected hot tub or pool until the rash has cleared and the water has been properly maintained. Shower with soap, change out of wet swimwear promptly, wash the swimsuit, and keep the affected skin clean and dry. Do not squeeze the bumps or shave across them because this can further irritate follicles.

If an antibiotic is prescribed, use it exactly as directed and do not share it. Tell the prescriber about medicine allergies, pregnancy, immune-suppressing treatment, and previous resistant skin infections. Seek reassessment rather than starting another product if the rash fails to respond as expected.

Arrange prompt medical advice if redness is spreading, pain or swelling is increasing, pus is collecting, the rash keeps returning, or you develop fever or feel unwell. Earlier assessment is sensible for a person with reduced immunity or a condition that makes skin infections harder to control.

Get urgent care for rapidly advancing redness, severe pain, confusion, faintness, breathing difficulty, or a significant decline in how you feel. These features need assessment for a deeper or more widespread infection rather than routine home care.

Ask whether the timing and follicle pattern fit hot tub folliculitis, whether a swab would change treatment, and what improvement should look like. If medicine is recommended, clarify why that route was chosen, how to use it, which side effects warrant stopping, and when the skin should be reviewed.
