How an antibiotic drug rash can appear

An antibiotic drug eruption may look like flat or slightly raised red, pink, purple or brown spots, depending on skin tone. It can be widespread, itchy or symmetrical. Hives are raised, itchy welts that may move between areas, while some antibiotics can make exposed skin unusually reactive to sunlight.

Timing varies: a reaction can begin soon after a dose, later in the course, or after the antibiotic has finished. A rash during treatment is not automatically an allergy; infection, another medicine or a separate skin condition can look similar. The pattern and accompanying symptoms help a clinician distinguish them.

What to do now

Note the antibiotic name, when each dose was taken, when the rash began and whether it is spreading. Take clear photographs in good light and list other new medicines, supplements, infections and sun exposure. This gives the clinician a useful timeline even if the appearance changes.

Seek professional instructions before changing treatment when symptoms are mild and stable, because the infection still needs an effective plan. Do not restart a suspected medicine later without medical advice. Avoid adding medicated creams or antihistamines until a pharmacist or clinician confirms they are suitable for you and will not obscure important features.

Emergency and urgent warning signs

Call emergency services for difficulty breathing or talking, throat tightness, wheezing, collapse, severe dizziness, or swelling of the lips, mouth, face or tongue. Use a prescribed adrenaline auto-injector as directed while help is being arranged if you carry one.

Obtain urgent assessment for blistering or peeling skin, painful skin, mouth or eye sores, a purple or bruise-like rash, fever with a spreading rash, swollen glands, or signs that several organs may be affected such as dark urine or yellowing of the skin or eyes. A rapidly changing rash also warrants urgent review.

Ask whether the appearance fits allergy, a non-allergic medicine reaction or another diagnosis; whether the current infection needs an alternative antibiotic; and how the reaction should be recorded.
