Anti-TNF treatment for ulcerative colitis

They are biologic medicines that block tumour necrosis factor, a chemical messenger involved in inflammation. A gastroenterology team may consider them when ulcerative colitis remains active despite other treatment or when a steroid-sparing option is needed.

Treatment aims to settle inflammation, improve symptoms such as diarrhoea and rectal bleeding, and maintain remission. Depending on the medicine, it may be given by an infusion in a clinic or an injection under the skin. Because TNF also helps the immune system respond to infection, blocking it can make some infections harder to control. Attend the requested screening and monitoring appointments, and give the team a complete list of medicines and supplements. Check vaccination needs before starting because live vaccines may not be suitable during treatment. Tell clinicians and dentists that you use an anti-TNF medicine.

Follow the prescribed schedule and do not change or stop doses on your own. If an injection or infusion is missed, contact the treatment service for specific instructions rather than doubling a dose. Report new infection symptoms before the next dose, and ask whether treatment should be delayed.

Contact the treating team promptly for fever, persistent cough, painful urination, shingles-like rash, an infected wound, unusual bruising, marked fatigue or worsening bowel symptoms. Seek urgent help for difficulty breathing, swelling of the face or throat, fainting, severe chest discomfort, confusion, new weakness, or a severe reaction during or after an infusion.

Heavy rectal bleeding, severe or increasing abdominal pain, a swollen abdomen, repeated vomiting, or inability to keep fluids down also warrants urgent assessment. These signs can reflect a significant ulcerative colitis flare or another complication and should not wait for a routine visit.

Ask which anti-TNF medicine fits your situation, how it will be given, how improvement will be assessed, and what the plan is if it does not control the disease. Clarify which blood or stool tests you need, how to handle infections or missed doses, and which vaccines should be completed before therapy.
